How to refer one of your patients to our programs or providers.
REFERRALS
If you have a patient you would like to refer to one of our programs or providers, please print out and complete our Referral Form and return it to us along with medical records or other appropriate documentation.
You may fax it to (206) 859-5031 or mail it to our clinic address. Please allow at least 2 business days for a coordinator to process your referral.
Once it is reviewed, we will secure the necessary authorizations (if required) and coordinate scheduling directly with the patient or representative.
Communication with you and other care providers is a top priority, and we will make sure you receive all appropriate reports and recommendations. If your patient is admitted to one of our treatment programs, we provide weekly progress reports and the opportunity to participate in weekly meetings.
It is not uncommon for referring providers or stakeholders to have questions related to the treatment provided here at RIW or the medical staff that will be providing care for their patient or client. If you have any specific questions or need assistance in facilitating your referral, please contact one of our referral coordinators at 206-859-5030 and choose option 2.
